How our clinic works

At Greenfield Medicine, you will find a streamlined process through which qualified patients will receive medical cannabis treatment in accordance with the laws of the State of Florida.

Upon contact, patients can expect to receive information about our practice, consent forms, and as well as communication for appointment scheduling. Consent forms must be completed and returned prior to the medical cannabis evaluation appointment.

Cannabis Certification Consultation

During an appointment, a physical evaluation will be conducted, including a full assessment of the patient’s medical history and confirmation that the patient has at least one of the qualifying medical conditions.

The assessment will determine if the use of medical marijuana would likely outweigh the potential health risks for treating the qualifying condition.

The medical marijuana use registry will be reviewed to confirm that the patient does not have an active physician certification from another physician.

The Medical Marijuana Use Registry will issue an ID number once the patient is entered into the Registry. Entering a patient into the Medical Marijuana Use Registry occurs after the consultation with Dr. Jara and all required medical documentation is received.

After being entered into the Registry, the patient and/or caregiver must submit a completed application to obtain a Florida Medical Marijuana Use Registry ID card. Applications are issued through the Office of Medical Marijuana Use. Once approved, an ID number and temporary ID card will be issued by email to the patient. Usually, within one month, a printed ID card is sent to the patient/caregiver, but only the temporary ID card is needed to purchase medical cannabis.

Obtaining medical cannabis from state-approved dispensaries

After receiving the temporary patient/caregiver’s state ID card, the patient/caregiver can then visit or call one of the Medical Marijuana Treatment Centers to arrange to get products sanctioned by the State of Florida.

Follow up appointments to stay active on the registry

To remain active in the Medical Marijuana Use Registry, a patient must rectify every 30 weeks. An appointment with Dr. Jara is required as part of every recertification consultation. To prevent a lapse in certification, a follow-up appointment should occur at least 1 week prior to the certification expiration date.
